Komen Foundation<\/a>, <a href=\"http:\/\/www.ywca.org\/site\/c.cuIRJ7NTKrLaG\/b.9360173\/k.1089\/YWCAEliminating_Racism_Empowering_Women.htm\">YWCA<\/a>, local Federally Qualified Health Centers and the\u00a0<a href=\"http:\/\/georgiacore.org\/default.aspx\">Georgia Center for Oncology Research and Education<\/a>. These partners offer community-based patient navigation support and a wide range of education and outreach services.<\/p>\n<p>A partnership with global nonprofit <a href=\"http:\/\/planetree.org\/\">Planetree<\/a> will allow GCCE to implement patient-centered services and earn recognition through the Planetree Designation Program. From 2010 to 2015, more than 80 percent of GCCE clients were African-American and one-third were uninsured.<\/p>\r\n\r\n<p> Many GCCE patients are diagnosed at a younger age (under 60 years old) and at a later stage in the disease than the national average. The most commonly diagnosed cancers among this group are breast, gynecological, lung and thoracic cancers. In addition, many GCCE patients have other diseases or medical conditions that complicate care.<\/p>\t\t\t\t\t\t  <\/div>\r\n\t\t\t\t\t\t<\/div>\r\n\t\t\t\t\t<\/div>\r\n\t\t\t\t\t<!-- Inner panel End -->\r\n\t\t\t\t\t\r\n\t\t\t\t\t\t\t\t\r\n\t\t\t\t\t<!-- Inner panel Start -->\r\n\t\t\t\t\t<div class=\"wpsm_panel wpsm_panel-default\">\r\n\t\t\t\t\t\t<div class=\"wpsm_panel-heading\" role=\"tab\" >\r\n\t\t\t\t\t\t  <h4 class=\"wpsm_panel-title\">\r\n\t\t\t\t\t\t\t<a  class=\"collapsed\"  data-toggle=\"collapse\" data-parent=\"#wpsm_accordion_560 \" href=\"javascript:void(0)\" data-target=\"#ac_560_collapse2\" onclick=\"do_resize()\">\r\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"ac_open_cl_icon fa fa-plus\"><\/span>\r\n\t\t\t\t\t\t\t\t\t\r\n\t\t\t\t\t\t\t\t \r\n\t\t\t\t\t\t\t\t<span class=\"ac_title_class\">\r\n\t\t\t\t\t\t\t\t\tINCREASING ACCESS TO HIGH-QUALITY CANCER CARE\t\t\t\t\t\t\t\t<\/span>\r\n\t\t\t\t\t\t\t<\/a>\r\n\t\t\t\t\t\t  <\/h4>\r\n\t\t\t\t\t\t<\/div>\r\n\t\t\t\t\t\t<div id=\"ac_560_collapse2\" class=\"wpsm_panel-collapse collapse \"  >\r\n\t\t\t\t\t\t  <div class=\"wpsm_panel-body\">\r\n\t\t\t\t\t\t\t<h3><strong>OUR APPROACH<\/strong><\/h3>\r\n<h4 style=\"color: #00877c;\"><strong>Improve timely access to and coordination of care for underserved patients<\/strong><\/h4>\r\n<ul>\r\n \t<li>Add clinically-trained nurses as patient navigators for breast\/gynecological cancer and lung\/thoracic cancer sites to guide patients through the complex aspects of cancer care, and to help coordinate the various components of care\r\n<ul>\r\n \t<li>Create a protocol to quickly link newly diagnosed cancer patients with a nurse navigator to help reduce the time between diagnosis and initiation of treatment<\/li>\r\n \t<li>Promote lay navigation services for all cancer patients<\/li>\r\n<\/ul>\r\n<\/li>\r\n \t<li>Ensure that patients complete all recommended therapies<\/li>\r\n \t<li>Enhance transportation resources for patients at high risk of missing appointments<\/li>\r\n<\/ul>\r\n<h4 style=\"color: #00877c;\"><strong>Enhance patient-provider communication and patient engagement<\/strong><\/h4>\r\n<ul>\r\n \t<li>Work with Planetree to create and implement a strategy to reach GCCE\u2019s patient-centered goals. With Planetree\u2019s assistance, GCCE will:\r\n<ul>\r\n \t<li>Conduct assessments to evaluate GCCE\u2019s current patient-centered culture and benchmark against the core principals of patient-centered care<\/li>\r\n \t<li>Coach and train GCCE clinicians in effective techniques to communicate a prognosis and treatment plans to patients with compassion, and to promote patient engagement in treatment decision making<\/li>\r\n \t<li>Establish a patient and family advisory board to gain input from patients and family members who have been through a cancer journey at Grady<\/li>\r\n<\/ul>\r\n<\/li>\r\n<\/ul>\r\n<h4 style=\"color: #00877c;\"><strong>Strengthen psychosocial and supportive care for underserved patients<\/strong><\/h4>\r\n<ul>\r\n \t<li>Develop low- or no-cost wellness programs, focused on nutrition and physical activity, to reinforce positive lifestyle choices that enhance quality of life and health outcomes.
